    This permit is intended for outdoor residential use only. This does not apply towards Special Events.

  3. Does the event take place within a 2 block radius of a place of worship?
  4. Have all neighbors been notified?*
    (All neighbors must be notified in advance)
  5. The Village will NOT approve an event if beyond 10pm

  6. Applicant Information

    I have fully read and agree to the provisions above and understand and agree to the assumption of risk, waiver, and release of all claims provisions of the outdoor sound permit policy.
